The New England Regional Spinal Cord Injury Center Model System and Spaulding-Harvard SCI Model System are partnering to present the Knowledge in Motion Education Series on November 29th, beginning at 6:30 PM EST. This webcast and live chat will discuss the community based Patient Centered Medical Home Model of providing primary care to people living with SCI, and efforts to expand models to coordinate care statewide and nationwide.
